Targeted Case Management
DBHIDS Targeted Case Management (TCM) is a service provided to adults and children in Philadelphia with mental health and/or substance use challenges. Services are available to individuals who need assistance accessing services, resources, and support to improve daily living.
Who is eligible for Targeted Case Management?
Individuals are eligible if they:
- Live in Philadelphia.
- Have a behavioral health diagnosis.
- Need assistance accessing behavioral health services and other resources.
- Have challenges that put them at risk of needing behavioral health hospitalization or other out-of-home care
Eligibility for the different levels of TCM service is based on the recent history and current needs of the individual.

Referrals
Anyone can make a referral for TCM services including individuals who feel they need the service, behavioral health providers, social workers, school counselors, etc.
Referrals can be made by completing the Philadelphia County Case Management Referral application. Click to complete an application. Each referral has a list of case management providers in the instructions.

Resource Coordination
Team of generalist case managers operating Monday through Friday for individuals who require less frequent contacts and support than BCM. The two specialty RC teams are: A shelter-based case management team and Prevention and ecovery Services (PARS), a short-term team of case managers which provides quick engagement, stabilization, and referrals to treatment and community support.
Intensive Case Management
A team of generalist case managers. These teams currently serve the homeless population and individuals who have a primary substance use disorder (SUD ICM).
Blended Case Management
A team of generalist case managers with the ability to provide intensive or non-intensive case management as the individual need dictates. These teams provide 24/7 service accessibility.
Community Treatment Team
Formally known as Non-Fidelity ACT/Blended Enhanced Case Management. A team of generalist case managers with a Nurse, Substance Abuse Specialist, and Psychiatrist (w/limited treatment capabilities) added as support to the program. A high intensity service recommended for individuals who experience frequent hospitalizations, crisis visits, and mobile emergency services. These teams provide 24/7 service accessibility.
Assertive Community Treatment
Assertive Community Treatment (ACT) is an evidence-based practice that provides community- based, multidisciplinary mental health treatment for individuals with severe and persistent mental illness who spend a significant amount of time receiving treatment in psychiatric inpatient hospitals. The goal of ACT is to lessen or eliminate the symptoms of mental illness that can impair an individual’s functioning and quality of life by providing the majority of treatment, rehabilitation, and support services that individuals need to achieve their goals and live independently in their community.
Children’s Case Management Services
Specialized case management services for children and adolescents dealing with serious mental illness and/or substance abuse issues. Intensive Case Managers are available 24/7 and generally see children with behavioral challenges.
ICM’s connect children and their families to needed mental health services as well as other community resources and supports. The ICM interfaces and advocates regularly with other relevant systems such as Intellectual disAbility Services, Juvenile Justice, Philadelphia School System and Department of Human Services.
