EPIC EBP Program Designation
The goal of the EPIC EBP Program Designation is to identify and roster providers who are offering high quality evidence-based and evidence-supported practices and to increase the number of individuals who receive evidence-based services. The EPIC EBP Program Designation outlines a set of standards that are expected for implementing an EBP Program and will enable CBH to set up mechanisms for monitoring and incentivizing the delivery of EBPs. Providers who receive the EBP Program Designation will be included on referral lists utilized by CBH Member Services, CBH Clinical Management and made available to behavioral health professionals, members, and the general public.

EBP Independent Practioner Designation
The EBP Designation has been expanded to include Independent Practioners within the CBH Network. Read the EBP IP Designation notification here. Applications may be requested by emailing EPIC_DBHIDS@phila.gov.
Applications will be accepted and reviewed on a rolling basis. Providers are encouraged to review the PDF of the application and other resource documents below to determine if their Program needs to be further developed prior to submitting an application. Please only apply when your program is prepared to meet all of the EBP Program Designation standards.

New applicants must register for a username and password prior to accessing the EBP Program Designation portal. Please store your login credentials in a safe place as you will need these credentials for future access to the portal. Additionally, if you have forgotten your username and password, do not re-register. Email EPIC at epic_dbhids@phila.gov to reset your login credentials.Log in to the EBP Program Designation Application portalAfter registering, this link will allow you to access the EPIC EBP Program Designation portal. You can use this link to create an initial application and to edit or update a previously submitted application. Please note: previously submitted applications can be updated by clicking on the original application in the portal home page, and selecting ‘edit’ at the bottom of the screen. Please do not create a new application when making updates or edits.
