Coordinated Consumer Services (CCS)
Coordinated Consumer Services is directly involved with the clinical care of participants. CCS provides client-specific follow up, activities, supports and services to mental health professionals and the mental health population in Philadelphia County. CCS is comprised of four distinct units. For more information on these units please click on the links below:
- Admission, Discharge and Planning Team (ADAPT)– is charged with “gatekeeping” admissions and discharges for the two Extended Acute Care Units (EAC) in the system and collaborates with others regarding admissions into Norristown State Hospital. The team provides screening, scheduling and assessment of all referrals to ensure appropriate level of care and utilization of resources.
- Behavioral Health Justice Related Services (BHJRS) seeks to reduce stigma, increase behavioral health awareness across systems, strengthen community and cross-system capacity, support recovery and resilience, and increase early intervention and access to behavioral health services. BHJRS mission is to address behavioral health and justice-related issues that impact the individuals served by DBHIDS.
- Specialized Clinical and Criminal Justice Unit (SCCJU) – this unit is responsible for all special client requests that might be directed to the Division from the Office of the Health Commissioner, CBH Clinical Director or other upper level management across the system. The Unit is also responsible for Mental Health Court and liaisons with other legal departments within the behavioral health system, including the Health Department, Public Defenders, State Hospital (NSH) and CHIPPS/SIPPS activity regarding legal issues and other specialized client requests.
- Targeted Case Management (TCM)– provides technical assistance and support to agencies across the county who provide the full array of case management services. This unit is focused on improving Targeted Case Management practices so that individuals with serious and persistent mental illness can access and participate in the services they need to cope with their illness and improve their quality of life.
- Transitions, Integration, and Partnerships (TIP)– this unit is responsible for the daily management of residential services funded by the Office of Mental Health. With a focus on admission, skills acquisition and discharge, TIP ensures residential services increase client outcomes, improve consumer choice and satisfaction, and make the most efficient use of funding and resources.
